What Is Phase I Orthodontic Treatment? 

At SPA Orthodontics, we believe early orthodontic care should be thoughtful, conservative, and child-centered. We recommend Phase I treatment when it truly benefits your child’s long-term oral health and development. If it is more advantageous to wait and start treatment at a later date, we are happy to monitor growth and guide you every step of the way. 

Phase I orthodontic treatment (also called early interceptive treatment) is orthodontic care provided to children before all permanent teeth have erupted, typically between ages 7–10. 

The goal of Phase I treatment is to guide proper jaw growth, create space for permanent teeth, and correct developing orthodontic issues early—before they become more complex and costly to treat later. Ultimately, Phase I treatment focuses on building a healthy foundation for your child’s growing smile. 

Who May Need Phase I Treatment? 

Phase I treatment is recommended for children who show early signs of orthodontic or jaw development concerns. A child may benefit from Phase I treatment if they have: 

  • Crowded or overlapping teeth 
  • Early or late loss of baby teeth 
  • Difficulty biting or chewing 
  • A narrow upper jaw or crossbite 
  • Protruding front teeth (increased risk of injury) 
  • Underbite or overbite 
  • Thumb sucking or oral habits affecting tooth position 
  • Speech or airway concerns related to jaw development 

The American Association of Orthodontists recommends an orthodontic evaluation by age 7, even if no problems are obvious. Early evaluations allow us to monitor growth and intervene when necessary. 

The Phase I Treatment Process 

1. Early Evaluation 

Your child’s orthodontic journey begins with a comprehensive exam, including digital imaging and an evaluation of jaw growth, bite alignment, and tooth eruption patterns. 

2. Customized Treatment Plan 

If Phase I treatment is recommended, we create a personalized plan based on your child’s specific needs. Treatment may include: 

  • Palatal expanders 
  • Limited braces on select teeth 
  • Space maintainers 
  • Habit-correcting appliances
  • Reverse Pull Headgear (worn at night for underbites)

3. Active Treatment Phase 

Phase I treatment typically lasts 6–12 months, depending on the complexity of the case. During this time, we guide jaw development and/or improve tooth positioning. 

4. Resting Phase (Monitoring Growth) 

After active treatment, appliances are removed, and your child enters a monitoring phase. During this phase a retainer may be recommended, and we will continue to track growth and tooth eruption until they are ready for Phase II (usually full braces or aligners if needed). 

Potential benefits of Phase I Treatment:

  • Helps guide proper jaw growth 
  • Creates space for permanent teeth 
  • Lowers the risk of impacted or crowded teeth 
  • Reduces the need for tooth extractions later 
  • Improves facial balance and bite function 
  • May shorten or simplify Phase II treatment (usually full braces or aligners)
  • Builds confidence during critical developmental years 

Early treatment may prevent more extensive orthodontic and jaw problems and reduce overall treatment time in adolescence. 

Will My Child Still Need Braces Later? 

Not all children who complete Phase I treatment will need Phase II treatment, but many do benefit from a second phase once all permanent teeth have erupted. 

If Phase II is needed, it is often shorter, more efficient, and less complex because early issues were already addressed.
